What type of tape is it, ceramic disc of standard washer?

If it's a standard type tap, does the tap turn the appropriate number of
turns?

It could be that the thread on the tap is "crudded" preventing
the
washer lifting far enough off the seat to allow full flow. Another
possibility I've seen a few times is the washer detaching from the
jumper and remaining on the seat. Water then flows through the hole in
the middle of the washer.

Either way, you need to strip down the tap to determine the cause - Just
don't forget to turn the water off first!
